Village Family Health Team, in Liberty Village

The Village Family Health Team’s (FHT) doors are open, in Liberty Village.

The Family Health Team (FHT) will have 7 physicians supported by a Nurse Practitioner, Registered Nurses, a Social Worker, a Dietitian and a Clinical Pharmacist, providing primary care core services: Health Assessments, Diagnosis and Treatment, Primary Reproductive Care, Primary Mental Health care, Primary Palliative Care, Service Coordination and referral, Patient Education/health promotion and disease prevention, Pre-post-natal care and obstetrical care, arrangements for around-the-clock care, Chronic Disease Management programs and Health promotion and disease prevention programs.

In addition to primary care core services, the FHT will have four priority programs: Pre-Diabetes and Diabetes, Child and Family Health, Mental Health and Addictions and Wellness Promotion, Illness Prevention and Screening.
